Admissions
We admit patients from acute and sub-acute care hospitals, long term acute care facilities, and sometimes from nursing homes. A Physician, an insurance company case manager, a social worker, a hospital discharge planner, a patient’s family member/advocate can refer patients. Our admission personnel can begin the admission process within 24 hours of receiving a referral.

Nursing Services
Nurses;
Our nurses play a major role in the care of our patients. Their role is not only limitted to medication administration but also to educate the patients and their love ones on the importance of being compliance to their Care Plan and treatment regimen.

PCTs/CNAs:
The pct/cna plays a crucial role in providing direct care to the patients. As an integral part of the care team, the pct/cna assist patients with essential daily activities including but not limited to bathing, grooming, eating, mobility etc.The dedication and expertise of our nurses and pct/cna ensure the smooth realization of the goals our patients set for
themselves.
Physician Services
Freedom Health Care System works in collaboration with physicians involve in the care of our patients to ensure continuity of care. We help set up their doctor’s appointments and arrange for transportation to and from their doctor’s office.
